Conditions We Treat
Sciatica is pain that travels along the sciatic nerve — from your lower back or buttock down the back of your leg, sometimes all the way into your foot. It is a symptom, not a diagnosis: the right treatment depends on what is irritating the nerve. At Mettle, the evaluation comes first, and your plan is built around the cause we find.
Symptoms
Causes
The sciatic nerve is the largest nerve in the body, formed from nerve roots in the lower spine. Sciatica happens when something irritates it along its path. The three most common sources:
A herniated disc or bulging disc pressing on a nerve root as it leaves the spine.
Spinal stenosis — age-related narrowing around the nerves, often worse with standing and walking, eased by sitting.
Piriformis syndrome — a deep muscle in the buttock irritating the nerve as it passes.
Sciatica-type pain can also persist or return after back surgery; that needs its own careful evaluation and surgeon-guided care — see post-surgical spine rehabilitation.
Telling Them Apart
Both can send pain down the leg, and they are often confused. Piriformis pain usually centers deep in the buttock, is worse with sitting on hard surfaces, climbing stairs, or rotating the hip, and typically stays above the knee. Disc-related sciatica more often travels below the knee, may bring numbness in a specific part of the foot, and changes as you bend or straighten your spine.
Your evaluation tests both: how your symptoms respond to positions and movements is one of the most useful clues. Mettle treats piriformis syndrome as well as sciatica — the point of the evaluation is finding out which one you have.
These can signal a condition that needs same-day medical care — not a therapy appointment.
Treatment
Every visit is one-on-one with a licensed physical therapist; your treatment is never passed to aides or assistants.
Getting Started
Your first visit is a full one-on-one evaluation, typically 30–60 minutes. Your therapist takes your history, tests movement, strength, reflexes, and how your symptoms respond to positions, then explains what they found and what it means. Most patients start treatment the same visit and leave with a short home program and a clear plan. Bring any imaging reports you have — they are helpful but not required — and wear clothes you can move in.
In New Jersey, you can start physical therapy without a physician referral. Some insurance plans set their own coverage requirements; we verify your benefits before treatment and explain what we find.
New patients can start with a free screening, and first visits are typically available within 24–48 hours. We verify your benefits before treatment. Medicare and most commercial insurance are accepted, and self-pay is available too.
Questions & Answers
It depends on the cause. Many people improve steadily over a period of weeks with consistent care; disc-related symptoms often take longer than a simple strain. We measure your progress in what you can do — sitting, walking, sleeping — and reassess at every visit. If you are not progressing, we will say so and refer you to a physician.
Usually not. The evaluation comes first; imaging is most useful when symptoms suggest something serious or when progress stalls. If you already have imaging, bring the report — and if you were diagnosed with a herniated disc, our herniated disc page explains how we approach it.
The honest answer is "it depends on the cause" — an exercise that calms disc-related sciatica can aggravate piriformis syndrome. That is why your program is built from your evaluation rather than a generic handout, and why it changes as your symptoms change.
Yes. Post-surgical sciatica is treated under your surgeon's protocol, with graded movement, nerve mobility work, and strengthening progressed as healing allows — see our post-operative care.
