Mettle provides one-on-one physical therapy in Basking Ridge and Martinsville, NJ. Fax your referral and we will contact your patient to schedule their evaluation — first visits are available within 24–48 hours, and benefits are verified before treatment begins.

Please include the referral order, the diagnosis, and any precautions or post-surgical protocols. If your office would like our referral packet, including a fax cover sheet, send us a note with the form on this page and we will send one over.

After You Refer

What Happens Next

Our team calls your patient to schedule their evaluation. We verify their benefits before treatment begins and review coverage with them, so cost questions are settled before the first visit.

Your patient is then treated one-on-one by a licensed physical therapist at every visit — treatment is never passed to aides or assistants.

Evaluation summaryFindings and the plan of care, sent after your patient’s initial evaluation.

Progress reportsUpdates during care and at re-evaluation — including progress reports to operating surgeons for post-surgical patients.
