Conditions We Treat

Physical Therapy for a Herniated Disc in Basking Ridge & Martinsville, NJ

A herniated disc sounds alarming — and the pain can be too: back or neck pain that may travel into an arm or a leg. But a herniation on an MRI is not an automatic sentence to surgery. Many herniations settle and improve with time and the right conservative care. What matters is matching treatment to how your disc problem actually behaves.

Understanding Your Scan

What the MRI words mean

  • Bulging disc: the disc's outer wall pushes outward but stays intact. Extremely common — and very often painless.
  • Herniated disc: material from the disc's softer center pushes through the outer wall, where it can irritate a nearby nerve.
  • "Slipped disc": the everyday term for the same thing. Discs don't actually slip.
  • Your scan is not the whole story: disc bulges and herniations appear on MRIs of people with no pain at all. We treat you and your symptoms — not just your images.

Symptoms

Does this sound familiar?

  • Back or neck pain, often sharper with bending, lifting, or prolonged sitting
  • Pain, tingling, or numbness traveling into the arm (neck disc) or down the leg (back disc)
  • Symptoms that ease or worsen with specific positions — an important clue we test at your evaluation
  • An arm or leg that feels weaker than it should

Get urgent medical care first if you have:

  • New loss of bowel or bladder control
  • Numbness in the saddle area
  • Weakness in an arm or leg that is getting worse
  • Symptoms that began with a serious fall or accident

These need same-day medical care. Our sciatica page lists the warning signs in more detail.

Conservative Care

The honest case for conservative care

Many disc herniations improve over time with conservative care: as the irritation around the nerve settles, therapy restores movement, strength, and confidence. That is not a promise, and it is not true for everyone — some herniations do need surgery. It is why, when no warning signs are present, a genuine trial of well-matched therapy is usually the sensible first step. And if we think a surgical opinion is the better next step, we will tell you that plainly.

Treatment

How Mettle treats a herniated disc

Every visit is one-on-one with a licensed physical therapist; your treatment is never passed to aides or assistants.

  • A one-on-one evaluation with a therapist with advanced training in spine rehabilitation, who tests how your symptoms respond to movement and builds an individualized plan around what calms them
  • Directional-preference exercise: we test repeated movements to find the direction that eases your symptoms, then build your exercises around it
  • Manual therapy for stiff, guarded segments
  • Nerve mobility work and positioning to reduce irritation along the nerve's path — part of our nerve and neuromuscular care
  • Graded exercise and trunk strengthening, loaded in stages
  • Activity coaching: how to keep working, lifting, and exercising safely while you recover, plus posture and ergonomic correction
  • If surgery is the right step: pre- and post-operative care — prehab beforehand and post-surgical rehabilitation afterward, following your surgeon's protocol

Neck Discs

Herniated disc in your neck?

A cervical (neck) disc herniation can send pain, tingling, or numbness into the shoulder, arm, and hand. Evaluation and treatment follow the same principles — see our page on neck pain and pinched nerves.

Getting Started

Your first visit

A full one-on-one evaluation, typically 30–60 minutes: your history, how and when symptoms started, movement and nerve testing, and a review of any imaging you bring. You leave with an explanation of what we found, a plan, and the first steps of your home program.

In New Jersey, you can start physical therapy without a physician referral. Some insurance plans set their own coverage requirements; we verify your benefits before treatment and explain what we find.

New patients can start with a free screening, and first visits are typically available within 24–48 hours. We verify your benefits before treatment. Medicare and most commercial insurance are accepted, and self-pay is available too.

Questions & Answers

Frequently Asked Questions

Can a herniated disc heal without surgery?

Many herniations improve substantially with time and conservative care, and symptoms often resolve even when the disc itself still looks abnormal on a scan. "Improve" is the honest word — outcomes vary, and no one can promise a result. Your evaluation tells us how your herniation is behaving and whether a therapy trial makes sense.

Should I rest until it goes away?

Almost never. Prolonged rest weakens the muscles that protect your spine and usually prolongs recovery. Graded movement is part of the treatment — we show you what is safe now and build from there.

What is directional preference?

Many disc problems have a direction of movement — bending backward, forward, or to one side — that eases symptoms and a direction that worsens them. Finding yours, through repeated-movement testing at your evaluation, lets us aim your exercises precisely instead of guessing.

I was told I need surgery. Should I still try physical therapy?

Often, yes — unless you have warning signs or worsening weakness, a trial of conservative care is commonly recommended first, and it does not close the surgical door. If you do have surgery, we provide prehab before it and rehabilitation after it, coordinated with your surgeon's protocol.

How long does recovery take?

It varies with the size and location of the herniation and how long symptoms have been present — typically weeks to a few months of progressive care. We track your symptoms and function at every visit so progress is measured, not assumed.

Do I need a referral to start?

In New Jersey, you can start physical therapy without a physician referral. Some insurance plans set their own coverage requirements; we verify your benefits before treatment and explain what we find.
