Conditions We Treat

ACL Reconstruction Rehab in Basking Ridge & Martinsville, NJ

Rebuilding a knee after ACL reconstruction is a months-long project — and who guides it matters. At Mettle, your rehabilitation is delivered one-on-one by a licensed physical therapist, following your surgeon’s protocol, and every stage progresses because you met objective criteria — motion, strength, control — not because a date on the calendar arrived.

Is This You?

Who this page is for

  • You’ve had ACL reconstruction, or it’s scheduled, and you’re choosing where to rehab
  • You’re weeks out from surgery and progress feels slower than you expected
  • You’re an athlete — or an athlete’s parent — asking when running, cutting, and sport become safe again
  • You tore your ACL and want to explore managing it without surgery (there’s a section for you below)

The Road Back

Rehab in phases — each one earned

Protocols vary by surgeon and graft, and your pace is set by healing and testing — but the shape of recovery looks like this. These phases are typical, not promised; you advance when your knee demonstrates it is ready.

  • Early phase: calm the swelling, fully straighten the knee, restore bending, wake the quadriceps back up, and walk normally again.
  • Strength phase: progressive strengthening of the quadriceps, hamstrings, and hips, with balance and neuromuscular-control training so the knee trusts itself again.
  • Running phase: a structured return-to-running progression, begun only when strength, motion, and control criteria are met.
  • Agility phase: agility, cutting, and sport-specific training, introduced in controlled stages.
  • Return to sport: objective return-to-sport testing — measured strength and functional criteria that must be met before clearance is discussed with your surgeon. Passing is earned, never assumed.

If a meniscus was repaired at the same time, its precautions are built into your protocol and respected at every phase.

Our Approach

Why criteria-based progression matters

A calendar can’t feel your knee. Returning to pivoting sport before strength and control are genuinely restored is how second injuries happen. Criteria-based progression means your therapist measures where you actually are — and advances, holds, or adjusts your program based on evidence from your knee, visit by visit. For some patients that feels slower; for others, faster. It is safer for everyone either way.

Before Surgery

Before surgery: prehab and a pre-operative consultation

Surgery goes better when you arrive stronger. Mettle offers bookable pre-operative consultations and prehab for ACL patients: restore motion, build strength around the knee, learn the crutch skills, precautions, and early exercises you’ll need afterward — so the first post-op weeks start ahead instead of behind. Choosing Your Clinic

Questions to ask any ACL clinic

Where you rehab is your choice — ask these questions of any clinic you consider, including ours. Our answers are in parentheses.

  • Will a licensed physical therapist treat me one-on-one at every visit — or will I be handed off? (At Mettle: Every visit is one-on-one with a licensed physical therapist; your treatment is never passed to aides or assistants.)
  • Do you follow my surgeon’s protocol, and will my surgeon hear how I’m doing? (Yes — rehab follows your surgeon’s protocol, and we send progress reports.)
  • How will you decide when I can run, cut, and return to sport — by dates, or by testing? (By objective criteria and testing.)
  • How often will you re-measure my strength and motion? (Regularly — progression decisions are based on measurements, not impressions.)

The Non-Surgical Path

Managing an ACL tear without surgery

Not every torn ACL is reconstructed. Some people — often called “copers” — return to their activities without surgery, with rehabilitation focused on strength, neuromuscular control, and sensible activity choices. Honest limits apply: knees that repeatedly give way, especially with pivoting and cutting, risk further injury to the meniscus and cartilage, and if yours keeps buckling, a surgical opinion is the right next step. Either way, the starting point is the same: an evaluation of what your knee can do today.

Contact your surgeon or get prompt care if:

  • You develop fever, increasing redness, or drainage around the incisions
  • You have calf pain or swelling (a blood clot must be ruled out)
  • The knee becomes suddenly much more painful or swollen without a cause
  • You fall directly on the knee

These are your surgical team’s territory — call them first, then tell us.

Patient Experience

From a Mettle patient

This review describes the experience of a Mettle patient recovering from ACL surgery.

After my ACL surgery, I was nervous about the long road to recovery. But thanks to the incredible care I received at Mettle PT, my rehab journey has been nothing short of amazing. From day one, the physical therapist Dolly was knowledgeable, supportive, and genuinely invested in my progress. She keeps me motivated and safe. Week by week, I see real improvements—not just in strength and flexibility, but also in my confidence. The clinic's positive atmosphere makes every session something to look forward to. Also I would like to mention the support of Luciane who helped me push further as I progressed, always keeping the sessions challenging but manageable. Their teamwork, attention to detail, and genuine care made all the difference. Now, I'm sure that soon I'll be back to doing the things I love—pain-free and stronger than ever. I highly recommend Mettle PT to anyone going through ACL rehab.

Getting Started

Your first visit

Bring your surgeon’s protocol and any operative report you have, plus your brace and crutches if you’re using them. Your therapist evaluates motion, swelling, strength, and how you walk, maps your plan to your surgeon’s phases, and sets the first goals with you. Sessions are one-on-one and typically 30–60 minutes. If your surgery is still ahead, your first visit can be the pre-operative consultation described above.

First visits are typically available within 24–48 hours, and new patients can start with a free screening. We verify your benefits before treatment. Medicare and most commercial insurance are accepted, and self-pay is available too.

Questions & Answers

Frequently Asked Questions

How long does ACL rehab take?

Longer than most people hope — returning to pivoting sport commonly takes 9 to 12 months, and that range exists for a reason: graft healing and restored strength cannot be rushed. Day-to-day function returns much sooner. Your timeline is set by your surgeon’s protocol and the criteria you meet, and we will always tell you honestly where you stand.

When can I start running?

When you meet the running-stage criteria — motion restored, quadriceps strength rebuilt to standard, good movement control — which for many patients is around the three-month mark. Meeting the criteria matters more than the date.

Do you communicate with my surgeon?

Yes. Your rehab follows your surgeon’s protocol, and we send progress reports so your surgical team knows how you’re advancing — including when return-to-sport testing is complete.

My surgery was at a hospital system. Can I still rehab at Mettle?

Yes. Bring your surgeon’s protocol and operative report, and we coordinate from there. Patients choose their outpatient clinic — you’re entitled to rehab where you’ll be treated one-on-one.

When can I drive?

That depends on which leg, your brace, your medications, and your surgeon’s guidance — it’s your surgeon’s call, and most patients ask at their first post-op check. We’ll help you plan around it in the meantime.

Do I need a referral to start?

In New Jersey, you can start physical therapy without a physician referral. Some insurance plans set their own coverage requirements; we verify your benefits before treatment and explain what we find. Most post-operative patients arrive with their surgeon’s protocol in hand, and that is always welcome.
